We are providing the form (linked below) from the National FOP Police Disaster Relief Foundation whereby an Active* member (their rules, not ours) can make a claim for assistance if he/she has suffered damage or loss caused by a disaster such as Hurricane Sandy.  The form should be filled out by the claimant and should be sent to the NYS FOP via fax (516 433 4473) or mailed to the NYS FOP, 911 Police Plaza, Hicksville, NY, 11801.  We urge all of our Active members who have suffered damage or loss, to complete and submit this claim form.  If you have any questions, contact NYS FOP Hqt's at 516-433-4455
